HCWP Care and Medication Attestation Summary

Regular clinic visits are required

Visit timing is set by your provider (usually every 6, 12, or 24 months)
Up-to-date visits are needed to continue prescriptions

If a visit is missed

A short-term prescription may be given until your next visit
Ongoing prescriptions require completion of required visits

Before surgeries or procedures

Contact HCWP at least 10 business days before
This helps us plan your care safely
Late notice may delay care planning

Medication supply

Always keep enough medication on hand
Order refills early, especially for regular use
Plan ahead for weekends and holidays (no shipping during those times)

Urgent care

For urgent needs, go to the Emergency Department (ER)
